Wellbeing Lincs

What

Service helping people stay independent through life's changes, including living with a long term health condition. People who meet at least four of the eligibility criteria will get a free tailored package of support, advice and guidance on housing, health and money management. Further services will incur a cost, such as getting someone's home adapted. Please see the website for more information and eligibility criteria.
