What

Community support service providing physical, practical and emotional support and care for people living with life-limiting conditions approaching end of life. It can provide treatment and advice to help manage symptoms, support people to make decisions for their future, work with loved ones and signpost to other services including financial advice. Who

Adults facing life-limiting illness who are registered with a Trafford GP, their loved ones and carers. Referral must be from a healthcare professional
