Exercise referral programme for people living with a long term medical condition such as cancer. It offers 12 weeks of subsidised access to a local leisure centre where people can swim and/or take fitness classes, use the gym and join weight loss support groups. A specific programme in this scheme helps people living beyond cancer move more and lead healthier lifestyles.
