Wirral Social Prescribing Scheme

What

Health and community project delivered by Citizens’ Advice Wirral and Age UK Wirral, to link individuals to a social prescriber. A social prescriber will work with individuals to co-design a personal action plan to improve their health and wellbeing. Activities can help with emotional wellbeing, mental and general wellbeing and levels of depression and anxiety. More information can be found on its website.
