Wellbeing Lincs- Supporting people to live independantly in their home and community

What

Service helping people stay independent through life changes, including living with a long term health condition. People who meet at least four of the eligibility criteria will get a free tailored package of support, advice and guidance on housing, health and money management. Further services incur a cost, such as getting someone's home adapted. Please see the website for more information and eligibility criteria.
