Hospital providing rehabilitation and palliative care for patients needing more support than home care but not acute hospital services. A team of doctors, nurses, occupational therapists and physiotherapists works closely with social services and care agencies to arrange placements and support. It also offers a Single Point of Access (SPA), which provides a range of community health services, including district nursing, hospital at home, falls prevention, speech and language therapy and end of life care.
