NHS service accepting referrals for people living with progressive life-limiting conditions. The service, open Monday to Friday from 8:30am to 4:30pm, offers pain and symptom management, end of life care, rapid discharge from hospital, advanced care planning, and support for complex psychological or emotional issues (patient/carer). Wigan. Team based at Royal Albert Edward Infirmary, although people are seen at each of the Trust’s sites

Who

People living with progressive life-limiting conditions. Any professional can refer, if patient consent has been obtained. Self referrals also accepted
