You must be referred by a health care professional directly to access this service, for example, by your District Nurse, Clinical Nurse Specialists or GP. It can help you identify eligible benefits, maximise your income, get support with appeals, tax credits, grants, housing costs, and transport concessions, as well as help completing benefit forms.
