Social prescribing service connecting people with wellbeing support in their local community. The social prescribing team are known as link workers. They discuss your personal needs with you and signpost you appropriately, connecting you with community groups, activities and services. Referrals to social prescribing can be made by health care professionals via their online referral form or you can self refer. See website and/or contact for more information.
